Gloucestershire Heath and Care

Children and young peoples’ services

You can request an autism assessment for a child or young person (0-18 years old) who lives in Gloucestershire if:

  • you are a professional who knows the child or young person, eg a SENDCO, mental health professional or paediatrician
  • the child or young person’s parent or carer agrees to an assessment (the child or young person can give consent if over 16)

You will need to gather information from home and school about the child or young person’s current difficulties with social communication, social interaction and evidence of any restrictive/repetitive behaviours that impact on their daily life.

Non-urgent advice: Guidance for parents

Gloucestershire’s Social Communication and Autism Assessment Service (SCAAS) and Child and Adolescent Mental Health Service (CAMHS) teams can accept referrals from any professional who knows the child well, however neither service accepts referrals from parents or self-referrals.

Speak to your child’s school or another health or social care professional who knows them well, and ask if a referral is appropriate to ensure they get the support they need.

Information from the child or young person’s parent or carer

First, ask the child or young person’s parent or carer to complete a family information form. You can do this through this service:

Alternatively, you can download a MS Word version and email it to the parent or carer so they can complete it on-screen or print it out. They will need to send the completed form to you as a PDF file so you can upload it when you request the assessment.

Before you start

You will need the child or young person’s:

  • home address and who they live with
  • school and GP details (name, address and contacts)
  • NHS number
  • current diagnosis and medication (if applicable)
  • supporting evidence for why an assessment is necessary and appropriate (you can upload this in PDF format)
